Privacy policy
How we collect, use and protect your information.
Elevate Health Clinic (ABGM Chiropractic Pty Ltd, ABN 93 603 438 017) handles your personal and health information in line with the Privacy Act 1988 (Cth), the Australian Privacy Principles and the Health Records and Information Privacy Act 2002 (NSW). This policy covers our Bella Vista and Earlwood clinics, our Mobile Exercise Physiology service and this website.
What we collect
- Your name, date of birth, address, phone number and email
- Medicare, private health fund, DVA, NDIS or WorkCover details
- Your health history, referrals, test results and treatment notes
- Emergency contact details
- Payment and billing records
How we collect it
- When you book online, by phone or in person
- From the intake forms you fill in before your first visit
- When you send us an enquiry through this website, email or social media
- From your GP, specialist or other health providers, with your consent
- During your appointments, including with our AI note-taking tool (see below)
AI note-taking (Heidi)
Some of our practitioners use Heidi, an AI medical scribe, to help write clinical notes. With your permission, it listens to and transcribes your consultation, and drafts a note that your practitioner checks, edits and approves before it’s saved to your record.
- Your practitioner will ask for your consent before using it
- You can say no, or ask them to stop at any time, and it won’t affect your care
- Your practitioner remains responsible for everything in your notes
Our website, bookings & cookies
- Online bookings are made through Cliniko, our practice management system. Your booking details go straight into your patient record.
- Enquiry forms on this website are processed by our web host, Netlify, and emailed to our reception team.
- Analytics and advertising: we use Google Analytics, Google Ads and Google Tag Manager, which use cookies to show us how people use the site and whether our ads lead to bookings. This data doesn’t include your health information. You can block or delete cookies in your browser settings, or opt out at tools.google.com/dlpage/gaoptout.
How we use your information
- To assess, treat and care for you
- To manage bookings, reminders, invoices and claims
- To communicate with your other health providers, with your consent
- To reply to your enquiries
- To meet our legal and professional obligations
We may send you appointment reminders and information about your care. If we send any marketing messages, you can opt out at any time. We will never sell your information.
Storage & security
Your records are kept in secure practice management software with access limited to staff who need it. Some of the service providers we use, such as our website host, Google and our AI note-taking provider, may store data outside Australia, including in the United States. We take reasonable steps to make sure they protect it.
Who we share it with
Only where needed for your care or required by law: your treating health providers, Medicare, private health funds, DVA, NDIS, WorkCover insurers, and the IT and software providers that help us run the clinic.
Accessing or correcting your information
You can ask to see or correct the information we hold about you by emailing info@elevatehealthclinic.com.au or calling (02) 8883 0178. We’ll respond within a reasonable time.
Complaints
If you have a privacy concern, please contact us first so we can try to fix it. If you’re not satisfied, you can contact the Office of the Australian Information Commissioner at oaic.gov.au or the NSW Information and Privacy Commission at ipc.nsw.gov.au.
